Full access to the Zori AI Medical Expert platform and Fountain Life app. Ask anything and get precise data-driven answers, track biomarkers over time, view trends, and manage health data in one place. Zori AI enables Members to securely store and organize their complete health history in one place. Members can upload prior medical records, laboratory results, imaging studies, and other clinical information, creating a longitudinal health record that extends far beyond a single annual exam.

The onboarding process

3-step online enrollment, without staff involvement.

Step 1

Personal information

The Member enters their name, date of birth, sex assigned at birth, Quest Diagnostics testing state, and mailing address. All fields are required before advancing.

Step 2

Legal review and waivers

Three legal documents must each be scrolled to the end before acceptance is unlocked: Terms & Conditions, Privacy Policy, and the BASE Membership Agreement. All three checkboxes must be checked to proceed.

Step 3

Secure online checkout

Members can optionally add targeted blood panels at additional cost before proceeding to a secure Stripe checkout. The checkout summary updates in real time as add-ons are selected.

A repeatable health baseline, not a one-time report

Members can purchase an additional BASE blood panel throughout the year to update their biomarker data, receive a new Review of Results through the Zori AI Medical Expert platform and renew their 12-month subscription to Zori AI and the Fountain Life App access.
